IMPORTANT: Following Public Health guidance, do not visit our office location to provide your evidence. Please upload all evidence via your online Housing Application and email housingoptions@bradford.gov.uk once you have done this.

To qualify for this band, you will need to provide all of the evidence listed below for scanning at one of our office locations. We are unable to accept incomplete documentation and you will be asked to return once you have obtained all of the evidence.

1. Evidence of eligibility for applicant and joint applicant. Click here to see a list of acceptable documentation.

2. Evidence of local connection. Click here to see a list of acceptable documentation.

3. A letter from you confirming the details of the person that you will be caring for, including:

• Their name
• Their address
• The reason that you need to provide care
• The type/nature of care you will be providing
• How often you will be providing care
• The reason that you cannot provide adequate care from your existing address

4. Evidence of the care needs of the person that you will be caring for from one or more of the following sources:

• A letter from their GP/consultant/other medical practitioner outlining the medical condition/health issue and why they require care
• A letter from Social Services outlining their medical condition/health issue and why they require care
• An Occupational Therapy report
• A letter from Age Concern or any other voluntary sector organisation representing them outlining their medical condition/health issue and why they require care

5. Evidence of the address of the person that you will be caring for from one or more of the following sources:

• Bank statement (dated within the last 6 months)
• Benefit award letter (dated within the last 6 months)
• NHS letter (dated within the last 6 months)
• Utility or other bill (dated within the last 6 months)
• Council Tax bill for the current financial year
• Child Tax Credit letter (dated within the last 12 months)
• Child Benefit letter (dated within the last 12 months)
• Letter from school/college/university (dated within the last 12 months)

6. Carers Allowance letter (if applicable)
